Position 1: Patient Care Coordinator (CCM / PCM / RPM)

Job Summary

Responsible for enrolling and managing patients under CCM, PCM, and RPM programs while ensuring proper documentation, follow-ups, and care coordination support. Responsibilities

Patient outreach and enrollment into CCM, PCM, and RPM programs Monthly follow-ups and care coordination calls

Maintain accurate EMR documentation

Coordinate with providers regarding patient care plans Support billing and compliance documentation

Requirements

Bachelor’s degree in a medical or healthcare-related field is required O/A Levels background is preferred

Strong English verbal and written communication skills

Experience in US healthcare, care coordination, or medical billing is preferred

Strong computer and documentation skills

Position 2: Behavioral Health Coordinator (BHI Program)

Job Summary

Supports patients enrolled in Behavioral Health Integration (BHI) by coordinating care, providing follow-ups, and ensuring proper documentation aligned with CMS guidelines. Responsibilities

Patient outreach and enrollment into BHI program Monthly behavioral health follow-up calls

Coordination with providers and behavioral health specialists

Maintain accurate clinical documentation

Assist patients in accessing behavioral health resources Requirements

Bachelor’s degree in Psychology or Social Work is required O/A Levels background is preferred

Strong English verbal and written communication skills

Experience in behavioral health, psychology, social work, or US healthcare is preferred

Strong interpersonal and documentation skills

Position 3: Quality Measures Coordinator (Care Gap Coordinator)

Job Summary

Responsible for identifying and closing preventive care gaps and ensuring patients complete required screenings and quality measures. Responsibilities

Identify preventive care gaps from patient records Patient outreach for screenings and preventive care

Coordinate appointments with providers and labs

Follow up to ensure completion of care gaps

Maintain accurate reporting and documentation

Requirements

Bachelor’s degree in a medical or healthcare-related field is required O/A Levels background is preferred

Strong English verbal and written communication skills

Experience in HEDIS, quality measures, or preventive healthcare is preferred

Strong organizational and communication skills

Position 4: Administrative Assistant

Job Summary

Responsible for providing administrative support, managing daily operations, maintaining records, and coordinating among internal teams, providers, and clients. Responsibilities

• Manage administrative tasks and daily office operations

• Maintain and organize documents, reports, and records

• Assist with scheduling, follow-ups, and coordination activities

• Communicate with internal teams, providers, and external contacts

• Prepare reports, spreadsheets, and documentation

• Track pending tasks and ensure timely completion

• Provide administrative support to ensure smooth workflow

Requirements

• Bachelor’s degree in any field is required (Healthcare or Business background is preferred)

• O/A Levels background is preferred

• Schedule: 11 AM to 3:00 PM EST

• Strong English verbal and written communication skills

• Proficiency in Microsoft Office (Word, Excel, Outlook)

Hiring Process

Shortlisted candidates will undergo a 10-day training and trial period. This training and trial period will be unpaid and is designed to assess communication skills, performance, and suitability for the role.

Probation Phase

Upon successful completion of training, candidates will move into a paid probation phase. Duration: Minimum 1 month

Working Days: Monday to Friday Working Hours: 2.5 hours per day

Timing: Between 10:00 AM to 7:00 PM EST (New York Time)

Total Weekly Hours: 12.5 hours per week

Salary: PKR 25,000 per month

Part-Time Role

Based on successful performance during probation, candidates may be transitioned to a part-time role
